🍏 iPhone / iPad

Open requests.hidescore.com in Safari (must be Safari)
Tap the Share button ↑ at the bottom
Tap Add to Home Screen — the name will say Jellyseerr; you can type Requests instead, then tap Add
Open the new icon → tap Sign in with Plex

🤖 Android

Open requests.hidescore.com in Chrome
Tap the ⋮ menu (top right)
Tap Install app (or Add to Home screen) — it may be named Jellyseerr
Open the new icon → tap Sign in with Plex

You stay signed in for a year — no re-typing the address, no logging in every time. Just tap the icon and request. Sign in with the same Plex account you use to watch.

Don't look for a "Requests" app in the App Store — there isn't one yet. Adding it to your Home Screen (above) is the way.
